Description
Care Management Practicum for Populations is the first of two practice for the care management program. In this course, students develop skills to transition to the care manager role for populations of patients within systems of care. The students will apply evidence-based didactic knowledge of care management and population disease/wellness care within healthcare settings. The focus of the course is application of evidence/research to planning and managing care for populations across systems over a life-span with attention to disease and prevention planning. Oversight for the practicum experience is provided by the instructor and the designated organizational partners.
